Support for Marylanders living with Sickle Cell Disease (SCD)

What is SCORE?

The goal of SCORE: Sickle Cell Outreach Resources and Engagement, is to advance health for Maryland’s sickle cell community by utilizing Community Health Workers (CHWs) to compassionately bridge gaps and reduce health inequities. SCORE is a compassionate, community-based program offering personalized support, resource connections, and care coordination for individuals and families impacted by Sickle Cell Disease (SCD) across all five Maryland regions.

What is a community health worker (CHW)?

CHWs are represented by a broad range of compassionate individuals who are respected liaisons for their communities. They have a deep understanding of the community’s culture, language, and priorities to serve as a link to or an intermediary between health and social services and the community to improve access to or the quality and cultural competence of service delivery. A CHW also builds individual and community capacity by increasing health knowledge, confidence, and self-sufficiency through a range of activities such as outreach, community education, the provision of information to support individuals in the community, social support, and advocacy.


What can our CHWs do for clients?

CHWs provide:

  • Client-centered care coordination
  • Resource navigation
  • Referrals to SCD-specialized and social services 

CHWs help clients with:

  • Access to hematologists and transfusion centers
  • Housing, food security, and transportation needs
  • Mental health counseling
  • Reproductive health and opioid-related services
  • Navigating social determinants of health
