By providing your mobile phone number and opting in to receive text messages from the Sickle Cell Outreach Resources and Engagement (SCORE) Program facilitated by HealthHIV, you consent to receive SMS (text) communications related to our organization and SCORE community health worker services.

By opting in, you may receive text messages regarding:

• Transportation coordination and ride updates
• Follow-up care reminders
• Case management communications

HIPAA & Privacy Considerations

HealthHIV is committed to protecting your privacy and complying with the Health Insurance Portability and Accountability Act (HIPAA).

Text messaging is not always a secure form of communication. While we take reasonable safeguards to protect your information, SMS messages may not be encrypted and could potentially be accessed by unauthorized individuals.

By opting in to receive text messages, you acknowledge and accept the potential risks associated with SMS communications.

We will make reasonable efforts to limit the amount of Protected Health Information (PHI) included in text messages. Sensitive or detailed medical information will not be shared via SMS.

Message and data rates may apply based on your mobile carrier’s terms and your individual mobile plan. HealthHIV is not responsible for any charges incurred by your carrier.

Opt-Out Instructions

You may opt out at any time by texting STOP

After you send “STOP,” you will receive a confirmation message and will no longer receive SMS messages from HealthHIV unless you re-enroll.
