SCORE Stakeholder Engagement materials are downloadable, print-ready palm cards and flyers designed to educate providers and individuals in Maryland. Use these materials to share information about the SCORE program and find answers to common questions about referrals, eligibility, and the program.
Provider Palm Card
A detailed, two-sided resource for healthcare providers. Available in a convenient size, these palm cards provide information for patients living with sickle cell disease, the no-cost services SCORE CHWs provide, and how to access the program.

Frequently Asked Questions
HIPAA guidelines explicitly permit providers to share Protected Health Information (PHI) with outside providers without patient authorization for the purpose of treatment, consultation, and care coordination. This includes sharing medical records, medication histories, and test results for continuity of care. (45 CFR 164.506)
Feedback is in writing at the end of the six (6) month CHW/Client relationship. However, if providers prefer more timely feedback, periodic case conferences can be scheduled. Providers can reach out to the SCORE CHW Manager, [email protected], to schedule those meetings.
- Any adult individual living with SCD can refer themselves to the program.
- Parents/Guardians of minor children living with SCD can refer their child.
Referrals are made via the program website, https://sicklecellchw.org.
The number and frequency of SCORE CHW/Client interactions varies widely based on the needs of the client.
- Any individual living with SCD who resides in Maryland is eligible, regardless of age or location in Maryland.
- Clients who are:
- In care but disengaged, e.g., missed appointments/low retention
- In danger of being lost to care
- In care but having difficulty navigating social support systems
A SCORE CHW will initiate contact, by phone or in person, with an eligible client within 3-5 business days of a referral from a medical provider, a program partner agency, a community partner, or self-referral.
The SCORE program uses the Brokerage Case Management model. This model supports a client leading the progress. The SCORE CHW checks in regularly and serves as a coordinator of care and client services with an emphasis on the positive accomplishments and attributes of the client. In this model, SCORE CHWs guide clients in assessing both internal and external resources that promote the client’s health and wellness to focus on actionable steps towards accomplishing client-identified goals that lead to improvement in overall health and wellness.
After the initial six (6) months, one of the following will occur:
- A full release from the SCORE program with encouragement for self-management,
- A referral to an SCD medical case management program, or
- A six-month extension of SCORE CHW services followed by a re-evaluation.
This program is funded wholly or in part by the Office of Children and Youth with Specific Health Care Needs, Maryland Department of Health, Prevention and Promotion Administration.
